Course Overview:

Have you ever wondered how serious incidents can, unfortunately, take place?

In this 2-hour session, we will discuss a range of serious incidents that have occurred locally in recent years, and start to think analytically, in order to understand what went wrong. We will review some basic Human Factors content, before exploring cases together, recognising what went wrong, and how those incidents could have been avoided. These sessions are facilitated by a clinical staff member, up to consultant level.

Learning Objectives:

  • Review basic Human factors concepts
  • Discuss cases where an incident occurred and apply Human Factors thinking
  • Think about how we can improve our systems and areas to avoid future serious incidents

Target Audience:

All staff- medical and non-medical. No prior Human Factors training required.
