Reducing Delays for Hospital Discharge and Transfers of Care.

Brief overview:

These sessions will be delivered by Carol Martin, Strategic System Lead for Transfers of Care and Christina Hartley, Operational Lead for Discharge in conjunction with Joanne Richardson, Improvement Manager – Therapies, Emergency Care Improvement Support Team (ECIST), NHS England to provide the local and national picture and what we can all do to help.

The sessions will cover an update around Transfers of Care in South Tees across Health and Social Care and an exploration regarding risk and the services and support in place in the community for our patients.

Key learning outcomes:

‘Risk is an integral part of planning when considering if a person is ready to go home.’

The range of potential risks, and the interplay between home and hospital environments can mean that discharge planning with older people is a complex process, requiring collaboration between the ward MDT, community services, the person and their family.

How confident do you feel about taking calculated risks to ensure those you care for can go home?

Target audience:

This 2-hour session is a must for ward based Nurses and support staff, Ward Managers and Matrons, Therapy staff, Medical staff, Operational Managers, Single Point of access staff and community staff.
