To give insight into a selection of serious incidents that have occurred in recent years locally and provide an opportunity to analyse what went wrong by exploring human factors and consider how they could be avoided in the future.

Content
Teaching sessions based on looking at how we can learn lessons from serious incidents (SI’s) with a focus on the human factors. An insight into a selection of serious incidents that have occurred locally in recent years. An opportunity to analyse what went wrong by exploring human factors and consider how similar situations could be avoided in the future, A place to share their own experiences and thoughts for improvement of practice/processes to help enable us all to keep our patients safe from unintentional harm.
Certification
Once candidates have completed this course, will be provided with resources from the session and a certificate that is valid and can be used as part of your CPD.
