“Blame is the enemy of safety.” Sydney Dekker, 2017

As we continue to develop a more understanding approach to incidents the Trust is updating several key policies and adopting the new Patient Safety Incident Response Framework.

Alongside that we’re implementing an approach to incidents and adverse events that is more supportive and understanding; one that encourages organisational learning and leads to better care, for patients, staff, families and the Trust as a whole.

This training is essential for all line managers to understand our approach to resolution and their responsibilities within that process
